OUR HEALTH PROGRAMS
Since 2006, Heart To Heart has provided Home Health Aide and Personal Care Aide services through contracts with most of the Certified Home Health Agencies (CHHAs) and Long Term Home Health Care Programs (LTHHCPs) in New York. Heart To Heart flagship division of Home Care has Accreditation with Commendation from the Joint Commission on Accreditation of Healthcare Organizations.
Social Adult Day Care
Everyone ages. It is an inherent property of life. As our bodies and minds develop over time, we adapt to these changes by modifying our decisions and actions.
Support Services
Heart to Heart operates Residential Group Homes for individuals with intellectual/developmental disabilities.
Behavioral Health
Heart to Heart’s behavioral support services promote results-oriented behavioral support for individuals and their families through customized behavior support plans.
Support Coordination
Support coordination is crucial for finding and coordinating the necessary services, resources and supports within the community that are required by adults and children with intellectual and/or developmental disabilities and their families.
Skilled Nursing
Nurses work with the patient’s medical team to maintain the patient’s safety and well-being in their home. A particular tribute to the attentiveness of the nursing staff is the excellent record that Heart to Heart enjoys with regard to patient safety and quality care.
