RESIDENTIAL PROGRAM
Our Program
Heart to Heart operates Residential Group Homes for individuals with intellectual/developmental disabilities. Our homes are beautifully equipped with comfortable amenities, private spacious bedrooms, and warm inviting living areas for all of our residents to enjoy together. Our homes are accessible and are custom designed to meet the needs of each and every individual. All programs and homes are staffed 24/7 with caring and nurturing Direct Support Professionals (DSP).
Heart to Heart’s residential programs are platforms in which individuals can learn, grow, and develop relationships with others in the community. We provide a safe, comfortable, and family-like environment for all of the individuals in our programs. Our residents are supported by trained Direct Support Professionals (DSP) daily in their homes and in the community to magnify and develop independence. Supervision and oversight of health and safety wellness are also provided to each individual in the residential programs.
We provide transportation to and from medical appointments and are constantly maintaining communication with families and authorized persons. We are aware of the importance of following up with physicians’ orders and carrying them out to aid in the care of our residents. Individuals in the programs will learn social integration, attend community events, and remain active within the community.
Our support staff (DSP) transport individuals to leisure activities such as shopping, recreational, and social activities. Heart to Heart’s residential programs are platforms in which individuals can learn, grow, and develop relationships with others in the community. We aim for independence and mentor individuals to make decisions that are best suited for them, along with the involvement of family and authorized persons who are involved in the plan of care.

Our clients gain independence with the assistance and supervision provided by our staff through structured daily living. Families, guardians, and caregivers will be informed and kept up-to-date of all medical appointments, follow up consultations and any changes. Clients in our residential programs will gain social and communication skills. We encourage our residents to participate in community events, which promote productive citizens. We also encourage family involvement that can build better relationships between residents and staff.
Family Involvement
Family involvement and participation is always welcomed and encouraged. We pride ourselves on working closely with families, residents, and their teams and support involvement on all levels. Coordinating doctor’s appointments, accommodating special diets, and arranging special visits and trips are all part of ensuring that individuals and families are comfortable, involved, and connected at all times.
