Supported Employment
Heart to heart Supported Employment Programs assist individuals with Intellectual and Developmental Disabilities (IDD) in obtaining and maintaining employment.
Prevocational Training
Heart to heart Prevocational training assists individuals with IDD focus on developing good work habits and gaining the basic skills and abilities essential for employment in any field.
Respite
Respite care provides short-term relief for family/primary caregivers of individuals with Intellectual and Developmental Disabilities (IDD). Respite can be arranged for an afternoon or for several days or weeks. Care can be provided in the participant's home, or in a host family home.
Individual Supports
Heart to heart Direct Support Professionals work closely with individuals, families, and program specialists to ensure all individuals live their life as they choose. Working 1:1 with individuals with Intellectual and Developmental Disabilities (IDD) to meet the goals set for them by their collaborative care teams.
Community Based Supports
Providing direct support and assistance for Individuals with Intellectual and Developmental Disabilities (IDD) to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community.
