How to Apply for New York’s NHTD Program

Applying for the NHTD Medicaid Waiver Program is simple, but the process can feel overwhelming without the right guidance. Here’s how to get started:

  • Confirm Eligibility: The program is for individuals who are Medicaid-eligible, need a nursing home level of care, and wish to remain in their home or community.
  • Contact a Regional Resource Development Center (RRDC): The RRDC will help guide you through the application, assessment, and approval process.
  • Create a Personalized Service Plan: Once approved, a care manager will work with you to design a service plan that fits your needs and goals.
  • Start Receiving Services: After your plan is finalized, services can begin — helping you or your loved one stay safe, healthy, and independent at home.
